Fabletics Grant Program Official Rules

TERMS AND CONDITIONS OF AWARD

ALL APPLICATIONS AND GRANTS ARE SUBJECT TO THE POLICIES OF FABLETICS, A DIVISION OF JUST FABULOUS, INC. (“FABLETICS”).

1.         ELIGIBILITY

The Fabletics Grant is only open to women who are at least 18 years old and legal residents of the United States.  By applying for the Grant, you represent and warrant that you are a woman who is at least 18 years old, who is a legal resident of the United States, and that all information you submit is accurate and truthful. Official employees of Just Fabulous, Inc. are prohibited from applying.

2.         GRANT APPLICATION PROCESS

A.    Application Submissions. Grant applications may be submitted beginning Tuesday, October 15, 2013, through the Grant Program App on the Fabletics Facebook fan page. Applicants must submit a video that is between two (2) to three (3) minutes in length, or submit an essay that is no more than two hundred and fifty (250) words with an accompanying photograph. The application submission should prominently feature the applicant and her concept.

B.     Guidelines for Grant Applications.

    1. The Grant application should clearly outline how the applicant’s project will create positive change in the area of health and wellness in her community;
    2. The Grant application should address why the applicant is passionate about this project; and
    3. The Grant application should state how the applicant will use the Five Thousand Dollars ($5,000.00) award to directly impact the health and wellness of others in her community.

 C.  Deadline for Grant Applications.  All Grant applications must be submitted by Friday, November 8, 2013.

 D.  Judging of Grant Applications. Fabletics Co-Founder, Kate Hudson, along with the Fabletics Team will evaluate all Grant applications in order to choose eight (8) finalists.  Each Grant application will be judged on its adherence to the Guidelines for Grant Applications described above, the potential social impact of the applicant’s proposed project, and the passion expressed by the applicant.

 E.   Notification to Finalists. Fabletics will notify eight (8) finalists via email by Wednesday, November 13, 2013. Each Finalist must accept their position within forty-eight (48) hours of receipt of the Notification Email.  Each finalist will be required to provide a more detailed overview of her concept proposal, including resource allocation, project timelines  and intended beneficiaries of the project.  Each finalist may be asked to review her proposal via telephone with the Fabletics Team.

F.  Selection of Recipients. Fabletics Co-Founder, Kate Hudson, along with the Fabletics Team will review each finalist’s detailed submissions and select four (4) winners of the Five Thousand Dollar ($5,000.00) Fabletics Grant.  Each finalist will be judged on the quality and completeness of her concept proposal, the potential social impact of her proposed project, and the project’s likelihood of success.

G.  Notification to Winners. Winners will be contacted by Monday, December 16, 2013.

3. GRANT PAYMENT INFORMATION

A. Payment. Each Grant recipient will receive a payment of Five Thousand Dollars ($5,000.00). The Grant payment shall be made after the recipient completes all forms and paperwork required by Fabletics.  Grants are subject to revocation, termination and suspension as outlined in the rules herein.

B. Taxes. Each recipient is responsible for paying all applicable taxes associated with the Grant payment.  Fabletics requires that each recipient submit an IRS W-9 form prior to payment.

4. RECIPIENT OBLIGATIONS

A.  Orientation. Each recipient must make herself available to complete a phone orientation with the Fabletics Team.

B.  Agreement. Each recipient must sign an agreement obligating her to use the Grant payment in the manner outlined in her Grant application. Each recipient will also be required to:

i.  Submit a blog post or a one (1) minute video outlining the progress made on her project every two (2) weeks following receipt of the Grant payment; and

ii.  Submit a two (2) minute video and an additional blog post by Monday, March 10, 2013, detailing the public launch of her project; and

iii.  Submit a blog post or one (1) to two (2) minute video featuring the impact of her project on the community three (3) months following the launch of her project.

5. FAILURE TO COMPLETE GRANT PROJECT  

A.  Failure to execute and publicly launch the project as outlined in the Grant application for any reason whatsoever by Monday, March 10, 2014, shall result in the revocation of the Fabletics Grant and recipient shall reimburse the entire Grant payment. Fabletics reserves the right to pursue any and all legal remedies to pursue reimbursement of the Grant payment.

6. PUBLICITY AND PUBLIC USE

A.  Public Announcement. Recipients first name and the nature and geographic location of their project, may be publicly announced by Fabletics.

B.  Use of Recipient’s Submitted Materials. Fabletics reserves the right and permission to use, reproduce, edit, distribute, publish, broadcast and display in perpetuity, any and all videos, photography and written materials, including the name, image, picture, portrait, and likeness of Grant recipients submitted by Grant recipients related to the Fabletics Grant Program, in any form of media anywhere in the world, without any further consideration or compensation due to Grant recipients.
